Acetaminophen, Codeine, Doxylamine falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Onion interacts with.

Theoretically, taking onion might increase the levels and clinical effects of drugs metabolized by CYP2E1.
Animal research shows that taking onion powder inhibits CYP2E1. However, this interaction has not been reported in humans.

Here's the practical picture with Mersyndol (which combines acetaminophen, codeine, and doxylamine). The piece of this combo that touches the enzyme in question (CYP2E1) is the acetaminophen. That enzyme normally turns a small slice of acetaminophen into a byproduct that can stress the liver. Lab and animal work suggests onion compounds may slow CYP2E1, which in theory could shift how acetaminophen is processed. Importantly, this has not been seen in real people, so it's a theoretical concern, not a proven one.

Everyday cooking amounts of onion are very unlikely to matter. Concentrated onion supplements are where I'd pause. The bigger everyday caution with this product is keeping total acetaminophen in check and watching for codeine's drowsiness. If you're using onion capsules medicinally, just run it by your pharmacist or doctor first.
