Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know if you're using Levemir (insulin detemir) and thinking about onion supplements. Onion itself can gently lower blood sugar, thanks to compounds like quercetin and its sulfur compounds. Since insulin detemir is already working to bring your glucose down, adding a concentrated onion supplement on top could push your sugar a little too low.

That means you might notice more signs of low blood sugar (hypoglycemia), like shakiness, sweating, dizziness, or feeling foggy. The good news: the evidence here is fairly limited, and eating onions as a normal part of meals is very unlikely to cause trouble. Concentrated supplements are the bigger question. If you'd like to try them, check with your pharmacist or doctor first and keep an eye on your glucose readings.

“Anyone using insulin detemir (Levemir) should treat this list with real caution, because nearly every one of the 242 ingredients on it acts on blood sugar, and most stack extra glucose-lowering on top of the insulin you already inject. Onion is a common food with a long history in traditional medicine, and it contains antioxidants like quercetin and sulfur compounds. Eating onion as part of a balanced diet is safe for most people, but high-dose supplements have only limited human evidence for any specific health condition. Talk with your pharmacist or doctor before using concentrated onion products for a medical reason.
