Itraconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Oregon Grape interacts with.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, moderately inhibits cytochrome P450 3A4 (CYP3A4).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two. Itraconazole is cleared from your body mainly by a liver enzyme called CYP3A4. Oregon grape contains berberine, which lab studies suggest can slow down that same enzyme. If berberine puts the brakes on CYP3A4, your body may not break down itraconazole as quickly, so its level could creep up. That could mean a higher chance of itraconazole's usual side effects, like nausea, stomach upset, headache, or strain on the liver.
I want to be honest though: this concern is theoretical, based on how things behave in the lab, not on real cases. Still, since itraconazole already affects CYP3A4 itself, it's worth a quick chat with your pharmacist or doctor before combining them. Please don't stop or change anything on your own.

Here's the basic idea with Oregon grape and your itraconazole. Itraconazole is carried in and out of cells partly by a pump called P-glycoprotein (P-gp). The berberine compounds in Oregon grape can slow that pump down, at least in lab dishes. If the pump moves less drug out, a bit more itraconazole could stay in your system. That theoretically means somewhat higher blood levels and a greater chance of side effects like nausea, stomach upset, headache, or rarely changes in liver values.
I want to be honest about the strength here: this is based only on test-tube research, not real people, so it's more of a heads-up than a proven problem. Itraconazole is not a prodrug, so the direction is toward more drug, not less. Still, check with your pharmacist or doctor before combining them.

Oregon grape is a shrub whose root contains berberine and related compounds. The strongest (though still modest) evidence is for topical creams that may slightly ease psoriasis; evidence for other uses is limited. Talk with your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
About Itraconazole
What Is Itraconazole Used For?
Brand names: Sporanox, Sporanox Injection, Tolsura
Itraconazole capsules (including Sporanox capsules and generic itraconazole capsules) treat serious fungal infections — blastomycosis, histoplasmosis, and aspergillosis — in both people with normal immune systems and those who are immunocompromised. Capsule forms are also approved for nail fungus (onychomycosis) caused by dermatophytes in non-immunocompromised patients. Tolsura (a gelatin-coated capsule formulation) treats blastomycosis, histoplasmosis, and aspergillosis in adults, but is not approved for nail fungus and is not interchangeable with other itraconazole products.
Itraconazole oral solution is specifically approved for oropharyngeal candidiasis (thrush — a fungal infection of the mouth) and esophageal candidiasis (a fungal infection of the food pipe). Only the oral solution has been shown to work for these mouth and throat infections; capsules cannot be substituted for this use.
