Bazedoxifene Acetate, Conjugated Estrogens falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the situation with Duavee (conjugated estrogens plus bazedoxifene) and Panax ginseng. Both parts of Duavee are partly broken down by a liver enzyme called CYP3A4, and ginseng can nudge that enzyme in either direction. That makes the effect hard to predict. In some studies ginseng sped up clearance of CYP3A4 drugs (which could weaken the benefit, meaning more hot flashes or less bone protection), while in other cases it slowed clearance (which could raise estrogen-related effects like breast tenderness, bloating, or spotting).

The good news: this is mostly theoretical and based on limited research, not strong evidence that it harms people on Duavee. Still, I'd run it by your pharmacist or doctor before combining them, rather than guessing.

Laboratory research and some case reports suggest that Panax ginseng can have estrogenic effects due to competition for estrogen receptors. The estrogenic activity is attributed to the ginsenoside constituents of Panax ginseng.

Here's the thing with Duavee (your conjugated estrogens plus bazedoxifene combo): Panax ginseng contains plant compounds called ginsenosides that can act a little like estrogen by latching onto the same receptors in your body. In theory, taking large, concentrated ginseng doses alongside your hormone therapy could nudge that estrogen activity, possibly affecting how well your treatment controls hot flashes or works for your bones, or adding to estrogen-type effects.

I want to be honest about the strength of this: it's theoretical, based on lab studies and a few case reports, not solid human trials. An occasional cup of ginseng tea is unlikely to matter much, but daily high-dose supplements are a different story. Before adding ginseng, just run it by your pharmacist or doctor so they can keep an eye on things.

What Is Estrogen and Bazedoxifene Used For?

Brand name: Duavee

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
