Selpercatinib falls under QT INTERVAL-PROLONGING DRUGS — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng has an additive effect with drugs that prolong the QT interval and potentially increase the risk of ventricular arrhythmias. However, research is conflicting.
Clinical research shows that short-term use of Panax ginseng can increase the QT interval. However, no changes in QT interval have been identified with prolonged use.

Here's the gist of pairing Panax ginseng with selpercatinib (Retevmo). Selpercatinib is already known to stretch out the heart's electrical recovery time, something doctors call QT prolongation. Ginseng may do a little of the same thing, so taking the two together could, in theory, add up and slightly raise the chance of an irregular heartbeat.

I want to be honest about the evidence: this is mostly theoretical, and the research is mixed. Some short-term studies showed a small QT increase with ginseng, while longer use showed no change. Still, since selpercatinib can already affect your heart rhythm, it's worth a quick chat with your oncologist or pharmacist before adding ginseng. Tell them right away if you feel dizzy, faint, or notice a fluttering heartbeat.

Selpercatinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the situation with Retevmo (selpercatinib) and Panax ginseng. Your body uses an enzyme called CYP3A4 to break selpercatinib down, and ginseng can nudge that enzyme in either direction. The honest answer is that we don't have a clear, consistent picture. In studies, ginseng sped up the clearance of one CYP3A4 drug (less drug around), but in another case it appeared to slow things down (more drug around). So the direction is genuinely uncertain.

That matters because selpercatinib isn't a prodrug, so higher levels could mean more side effects like high blood pressure, liver enzyme changes, or fatigue, while lower levels might mean it works less well against your cancer. Since this is a cancer therapy, please don't add ginseng without checking first. Run it by your oncologist or pharmacist.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
