Anagrelide falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Pau D'arco interacts with.

Theoretically, pau d'arco might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro research shows that pau d'arco reduces platelet aggregation and may interfere with vitamin K. One clinical study shows that taking the lapachol constituent of pau d'arco in doses above 1.5 grams daily increases the risk of bleeding. The effects of whole pau d'arco or pau d'arco extract in humans are unclear.

Here's the gist: anagrelide already lowers your platelets, which are the cells that help your blood clot. Pau d'arco may add a mild blood-thinning effect of its own, since lab studies suggest it can make platelets less sticky and possibly nudge against vitamin K. Stack those two together and you could, in theory, bruise more easily or bleed a little longer than usual.

I want to be honest about how solid this is: it's theoretical, based mostly on test-tube work, with only one human study (on a concentrated lapachol extract above 1.5 grams a day) actually showing more bleeding. We don't really know what whole pau d'arco does in people. Still, before combining them, run it past your doctor or pharmacist, and watch for nosebleeds, unusual bruising, or blood in your stool.

Anagrelide — sold as Agrylin and as generic anagrelide capsules — is used to treat thrombocythemia secondary to myeloproliferative neoplasms. That means it's prescribed when a bone marrow disorder causes the body to produce too many platelets, raising the risk of dangerous blood clots.

By bringing platelet counts down to a safer range, anagrelide also helps reduce related symptoms, including bleeding and clotting events (thrombo-hemorrhagic events).
