Dipyridamole falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Pau D'arco interacts with.

Theoretically, pau d'arco might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro research shows that pau d'arco reduces platelet aggregation and may interfere with vitamin K. One clinical study shows that taking the lapachol constituent of pau d'arco in doses above 1.5 grams daily increases the risk of bleeding. The effects of whole pau d'arco or pau d'arco extract in humans are unclear.

Here's the gist for someone on dipyridamole (Persantine). This medicine already works to keep your platelets from clumping together, which lowers your clotting risk after a heart valve replacement. Pau d'arco may push in the same direction. In lab studies its compounds (like lapachol) seem to reduce platelet stickiness and may also nudge against vitamin K. Stacked on top of dipyridamole, that could theoretically tip you toward easier bleeding, think more bruising, nosebleeds, bleeding gums, or cuts that take longer to stop.

I want to be honest: this concern is mostly theoretical and from lab work, with one study flagging higher lapachol doses (over 1.5 grams daily). The effect of regular pau d'arco supplements in people isn't well proven. Still, since dipyridamole is a clot-related drug, please run this by your pharmacist or doctor before combining them.

Dipyridamole tablets are used alongside warfarin (a blood thinner) to help prevent dangerous blood clots from forming after cardiac valve replacement surgery. It is not used alone for this purpose — it works as an add-on to anticoagulant therapy.

Dipyridamole Injection is used as an alternative to exercise during thallium myocardial perfusion imaging — a type of heart scan that checks blood flow through the coronary arteries. It is given to patients who are unable to exercise adequately for the test.
