Pennyroyal has a moderate interaction with acetaminophen, dextromethorphan, doxylamine. Concomitant use of pennyroyal essential oil and acetaminophen might increase the risk of acetaminophen-induced hepatotoxicity.

Here is the interaction we found between these two, with the evidence behind it.
ACETAMINOPHEN (Tylenol, others)
Pennyroyal is hepatotoxic and depletes hepatic glutathione levels in a similar manner to acetaminophen. A case report describes hepatic failure in a female who took pennyroyal and acetaminophen concurrently.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of NyQuil I want to focus on here is the acetaminophen (the Tylenol part). Your liver uses a protective molecule called glutathione to safely clean up acetaminophen's breakdown products. Pennyroyal oil works against you in two ways: it is directly toxic to the liver, and it also burns through that same glutathione supply. So pairing the two can stack liver stress on top of liver stress.
There is an actual case report of a woman who developed liver failure after taking pennyroyal and acetaminophen together, so this is not just theory. Honestly, pennyroyal oil should never be swallowed at all. Please skip it and check with your pharmacist or doctor before combining anything with NyQuil.
