Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Peony interacts with.

Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.

Here's the idea behind this one. Your Evotaz combo (atazanavir plus cobicistat) is partly cleared by a liver enzyme called CYP3A4. In test-tube studies, peony seemed to slow that same enzyme down a bit. If that held true in people, your atazanavir levels could creep up, which might mean more of its typical side effects like nausea, yellowing of the skin or eyes (jaundice), headache, or stomach upset.

The honest truth is that this is theoretical. It's based only on lab data, not on real patients, so we can't say how strong it would be in your body, if at all. Since HIV therapy is something you really want to keep steady and predictable, I'd still loop in your pharmacist or doctor before adding peony rather than guessing.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
