Defibrotide is a powerful clot-fighting medicine given in the hospital when someone develops liver vein blockage after a stem-cell transplant. Because it already works to keep blood from clotting, anything else that thins the blood could add to that effect. Peony root is one of those things to be careful with. In lab and animal studies, compounds in peony (like paeoniflorin) seem to slow clotting and reduce platelet stickiness.

Put them together and the worry is extra bleeding, such as easy bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding. Keep in mind this is theoretical, based only on test-tube and animal work, not human studies. Still, since defibrotide is used in fragile, high-risk patients, please clear any peony use with your transplant team first.

What Is Defibrotide Injection Used For?

Brand name: Defitelio

Defitelio (defibrotide injection) is used to treat hepatic veno-occlusive disease (VOD) — also called sinusoidal obstruction syndrome (SOS) — in adult and pediatric patients. This is a serious condition where small blood vessels in the liver become blocked, most often after a hematopoietic stem-cell transplant (HSCT).

Defitelio is specifically indicated when VOD is accompanied by kidney dysfunction or pulmonary (lung) dysfunction, making it a treatment for one of the most severe forms of this complication.
