Neostigmine Methylsulfate falls under CHOLINERGIC DRUGS — the group Phosphatidylserine interacts with.
Phosphatidylserine is thought to increase acetylcholine levels, which could theoretically lead to additive cholinergic effects when used with cholinergic drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Neostigmine works by raising acetylcholine, the chemical your nerves use to message muscles and glands. Phosphatidylserine is thought to nudge acetylcholine levels up too, so in theory the two could stack on top of each other and give you a bit more cholinergic effect than neostigmine alone.
If that happened, you might notice more of neostigmine's usual effects, things like extra saliva or sweating, watery eyes, an upset or crampy stomach, more frequent bathroom trips, or a slower heartbeat. I want to be honest though: this is a theoretical concern, not something well proven. Most people won't run into trouble. Still, since neostigmine is potent, mention the supplement to your pharmacist or doctor before combining them.
