Propolis and Beclomethasone dipropionate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Beclomethasone dipropionate

Beclomethasone dipropionate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the good news first: this is a very theoretical concern, and I wouldn't lose sleep over it. The idea is that propolis might block an enzyme called CYP3A4, which helps your body break down beclomethasone. If that enzyme slowed down, more of the steroid could linger in your system. With an inhaled or nasal product like Qvar or QNASL, the doses are small and act mostly right where you spray them, so this is even less likely to cause trouble.

In real life, this has never actually been reported in people, and animal studies showed no meaningful effect. If propolis levels ever did build up your steroid, you might notice things like a hoarse voice, mouth irritation, or oral thrush. If you use propolis regularly, just mention it to your pharmacist or doctor so they can keep an eye out.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Propolis

What is Propolis?

Propolis
  • Cold sores and herpes lesions (topical)
  • Sore throat and mouth sores
  • Wound and skin healing
  • Immune support
  • Oral and gum health

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. It is generally well tolerated, but allergic reactions can occur, especially in people allergic to bee products.

About Beclomethasone Oral Inhalation

What Is Beclomethasone Oral Inhalation Used For?

Brand names: Qvar, QNASL

Beclomethasone dipropionate HFA inhalation aerosol (brand: Qvar Redihaler) is used for the ongoing, daily management of asthma in adults and children 5 years of age and older. It is a preventive (maintenance) therapy — not a rescue inhaler — meaning it helps control asthma over time rather than treating sudden symptoms.

Important: Qvar Redihaler is not for the relief of sudden asthma attacks or acute bronchospasm. Always keep a separate fast-acting (rescue) inhaler on hand for those moments.
