Why this applies to Entrectinib

Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear entrectinib (Rozlytrek) out of your system. In some test-tube studies, propolis appeared to slow that same enzyme down. If that happened in real life, entrectinib could build up a little higher than expected, which might mean more of its usual side effects, things like dizziness, fatigue, swelling, changes in taste, constipation, or heart-rhythm changes.

The reassuring part is that this concern comes from lab and animal research only, and it has never actually been seen in people. Other studies found no effect at all. So this is more theoretical than proven, and everyday or food-level amounts are unlikely to matter. Still, because entrectinib is a serious cancer medicine, please run any propolis supplement past your oncologist or pharmacist first.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC) — a type of lung cancer where the ROS1 gene is rearranged — as confirmed by an FDA-approved test.

Rozlytrek is also approved for adult and pediatric patients older than 1 month with solid tumors that have an NTRK gene fusion (a specific genetic alteration), detected by an FDA-approved test, that have spread or cannot be removed without causing serious harm, and have worsened after prior treatment or have no good treatment alternatives. This NTRK indication was granted accelerated approval based on tumor response rates; continued approval may depend on results from confirmatory trials.
