Rosemary and Pentazocine

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Pentazocine

Pentazocine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rosemary interacts with.

Theoretically, rosemary might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that rosemary induces CYP1A2 enzymes. This effect has not been reported in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for combining rosemary with pentazocine (Talwin). Pentazocine is partly cleared by a liver enzyme called CYP1A2. In lab and animal studies, rosemary appears to speed that enzyme up a bit. If that happened in people, your body might break down pentazocine a little faster, which could leave you with slightly weaker pain relief.

The good news is this is only a theoretical concern. It has been seen in test tubes, not in humans, and it's rated as minor and unlikely. Everyday cooking amounts of rosemary are not a worry at all. If you're using concentrated rosemary supplements regularly and notice your pain control slipping, just mention it to your pharmacist or doctor so they can check in.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

From our pharmacist’s overview on the Pentazocine page

“Pentazocine (Talwin) is an opioid painkiller, so what matters most on this list is anything that deepens its sedation or slows your breathing, and here the risk is real enough to demand respect. Seventy-eight of the 167 supplement and herbal ingredients with an interaction on record are sedating in their own right, and six of the seven…”

About Rosemary

What is Rosemary?

Rosemary
  • Memory and concentration
  • Digestive upset
  • Hair growth
  • Anxiety and stress (aromatherapy)
  • Muscle and joint discomfort
  • Antioxidant support

Rosemary is a fragrant Mediterranean herb that is safe and flavorful in normal food amounts. Some early research suggests possible benefits for memory, mood, and hair growth, but the evidence is limited and far from conclusive. Concentrated supplements and the essential oil should be used carefully, and the essential oil should never be swallowed.
