Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rosemary interacts with.

Theoretically, rosemary might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that rosemary induces CYP1A2 enzymes. This effect has not been reported in humans.

Here's the gist for rosemary and ondansetron (Zofran). Your liver uses several enzymes to clear ondansetron, and one of them is CYP1A2. In test-tube studies, rosemary appears to speed that enzyme up a little. In theory, a faster enzyme could clear ondansetron a touch quicker, which might make it slightly less effective at keeping nausea and vomiting under control. Ondansetron is active on its own (not a prodrug), so this would lean toward a weaker effect, not a stronger one.

I want to be honest about how thin this is. It's only been seen in lab dishes, never in real people, and it's rated minor and unlikely. Rosemary in cooking is no concern. If you take concentrated rosemary capsules or oil, just mention it to your pharmacist or doctor.

About Ondansetron

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
