Caffeine (OTC drug) falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rosemary interacts with.

Theoretically, rosemary might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that rosemary induces CYP1A2 enzymes. This effect has not been reported in humans.

Here's the situation with rosemary and a caffeine pill. Your body clears caffeine using a liver enzyme called CYP1A2. In lab dishes, rosemary appears to speed up (induce) that enzyme. If that happened in real life, your body might break caffeine down a little faster, which could make the pick-me-up feel weaker or shorter than usual.

Caffeine works as-is, so it isn't a prodrug, no flip to worry about here. The big thing to know: this effect has only been seen in test tubes, never in people, and it's rated minor and unlikely. Normal cooking amounts of rosemary won't matter at all. If you take strong rosemary supplements regularly and notice a change, just check with your pharmacist or doctor.
