Abemaciclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Your body clears abemaciclib (Verzenio) mainly through a liver enzyme called CYP3A4. Lab studies hint that concentrated sage extracts might slow that enzyme down. If that happened in real life, abemaciclib could build up in your system and hit harder than intended, meaning more of its usual side effects like diarrhea, nausea, tiredness, low blood counts, or infection risk.

Here is the reassuring part: this is only a theoretical, test-tube concern and has not actually been seen in people. The amount of sage you'd sprinkle on dinner is very unlikely to matter. Concentrated sage capsules or essential oil are a different story. Since abemaciclib is a serious cancer drug, please run any sage supplement past your oncology team or pharmacist before adding it.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

About Abemaciclib

What Is Abemaciclib Used For?

Brand name: Verzenio

Verzenio (abemaciclib) is used to treat HR-positive, HER2-negative breast cancer in adults. For early breast cancer at high risk of recurrence, Verzenio is taken alongside endocrine therapy (tamoxifen or an aromatase inhibitor) after surgery to help prevent the cancer from coming back.

For advanced or metastatic breast cancer (cancer that has spread), Verzenio can be used in combination with an aromatase inhibitor as a first hormonal treatment, in combination with fulvestrant after the cancer has progressed on endocrine therapy, or alone after prior endocrine therapy and chemotherapy have stopped working.
