Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the gist for Calquence (acalabrutinib). Your body relies on an enzyme called CYP3A4 to clear this medication out. In lab studies, water-based sage extracts appear to slow that enzyme down. If that happened in real life, acalabrutinib could build up a bit higher than expected, which might mean more of its typical side effects, such as headaches, bruising or bleeding, low blood counts, or a higher chance of infection.

I want to be honest about the strength of this, though. So far this is only a test-tube finding, and it has never been reported in actual patients. Cooking with sage in your food is very unlikely to cause any problem. The bigger question would be concentrated sage capsules or oils. Since acalabrutinib is a serious cancer drug, please run any sage supplement past your oncologist or pharmacist first.

In vitro research suggests that common sage (Salvia officinalis) can inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for Calquence (acalabrutinib): your body uses a kind of cellular pump called P-glycoprotein to help move this medicine around and clear it. Lab studies hint that sage may slow that pump down. In theory, if the pump works slower, a bit more acalabrutinib could stay in your system, which might nudge up side effects like bruising or bleeding, headaches, low blood counts, or feeling more tired.

I want to be honest about the evidence, though. This has only been seen in test-tube studies, never confirmed in real people, so it may not amount to anything. A sprinkle of sage on dinner is very unlikely to matter. Concentrated sage capsules or essential oil are the bigger question mark. Since this is cancer treatment, please run any sage supplement past your oncology team or pharmacist before starting.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
