In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

This cold and flu combo includes dextromethorphan (the cough suppressant), which your body clears using an enzyme called CYP2D6. In lab tests, water-based sage extracts seem to slow that enzyme down a bit. In theory, that could let dextromethorphan build up, leading to more of its usual effects like drowsiness, dizziness, or feeling foggy. The chlorpheniramine (an antihistamine) can add to that sleepiness too.
Here's the honest part: this is only seen in a test tube, not in real people, so I wouldn't lose sleep over it. Everyday cooking amounts of sage almost certainly won't matter. Concentrated sage capsules or oil are where I'd be more cautious. If you take those regularly, just run it by your pharmacist or doctor first.

ANTICHOLINERGIC DRUGS

In vitro evidence suggests that common sage (Salvia officinalis) and Spanish sage (Salvia lavandulaefolia) can inhibit acetylcholinesterase and might increase acetylcholine levels.

Here's the piece that matters for your cold combo: the antihistamine in it, chlorpheniramine, works partly by blocking a brain chemical called acetylcholine. That blocking action is what helps dry up a runny nose, but it can also cause drowsiness, a dry mouth, and a bit of grogginess. Lab studies hint that sage may do the opposite, nudging acetylcholine levels back up.
In theory, that means concentrated sage could slightly blunt the drying effect of the antihistamine. Honestly though, this is only test-tube evidence, so I wouldn't lose sleep over it. A little sage in cooking or tea is very unlikely to matter. If you use strong sage extracts or the essential oil regularly, just mention it to your pharmacist or doctor.

Here's the piece worth knowing in your cold combo: the acetaminophen (the Tylenol part) is partly handled by a liver enzyme called CYP2E1. In animal studies, drinking sage tea seemed to rev up that enzyme. In theory, a faster enzyme could shift how acetaminophen is broken down. The honest truth is this has never actually been shown in people, so I'd file it under interesting but unproven rather than something to lose sleep over.
For everyday food and the occasional cup of sage tea, I wouldn't worry. Just don't lean on concentrated sage extracts or the essential oil alongside regular acetaminophen, and never exceed the labeled acetaminophen dose, since that's where real liver risk lives. If you use sage supplements often, run it by your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
