This combination cold-and-flu product has three ingredients, but the one to watch with sage is dextromethorphan, the cough suppressant. Dextromethorphan is cleared by a liver enzyme called CYP2D6. In lab tests, water-based sage extracts can slow that enzyme down. If that happened in your body, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or, rarely, feeling jittery or confused. The acetaminophen and pseudoephedrine parts are handled by other pathways and aren't really affected here.

The important thing: this is theoretical, based only on test-tube work, and hasn't been seen in real people. Everyday culinary sage in cooking is very unlikely to matter. If you regularly take concentrated sage extracts, just mention it to your pharmacist or doctor.

I want to be honest about the strength of this: it's based only on test-tube studies, and it has never actually been reported in real people. Sage as a seasoning in your food is very unlikely to matter. Concentrated sage extracts or the essential oil are where I'd be more cautious. If you regularly take a sage supplement, just mention it to your pharmacist or doctor so they can give it a quick look.

The dextromethorphan and pseudoephedrine parts aren't really the focus of this particular interaction.

I want to be honest about how thin this evidence is. It's only been seen in lab and animal research, never reported in actual patients, so it's very theoretical. Cooking with sage or sipping the occasional cup of tea is unlikely to matter. Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
