Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase levels of drugs transported by P-glycoprotein.
In vitro research suggests that common sage (Salvia officinalis) can inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the short version for Entereg (alvimopan). Your body uses a kind of cellular bouncer called P-glycoprotein to move alvimopan around and limit how much gets absorbed. In lab tests (test tube only, not in actual people), sage seems to slow that bouncer down. In theory, that could let a bit more alvimopan stick around in your system.

I want to be honest with you though: this is theoretical and based only on lab work, so it's a weak concern. Alvimopan is also usually given for just a few days in the hospital after bowel surgery, where the team is watching you closely. Cooking with sage is no issue. If you take concentrated sage extracts or the essential oil, just mention it to your surgical team or pharmacist.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Alvimopan Used For?

Brand name: Entereg

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
