This combination product (Tussend) already has a couple of ingredients that can make you drowsy. The hydrocodone is an opioid cough suppressant, and the chlorpheniramine is a sedating antihistamine, so together they can leave you feeling sleepy, foggy, or slowed down. The idea here is that some of sage's natural compounds may add a little extra calming effect on the brain, which could deepen that drowsiness.
I want to be honest about how soft this evidence is. It comes only from lab and animal studies, so it is theoretical. Using sage as a kitchen spice is very unlikely to cause a problem. Concentrated sage extracts or the essential oil are where I'd be more careful. If you take this medicine and use sage supplements, just check in with your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

Here's the wrinkle with this combo product (Tussend). One of its ingredients, hydrocodone, is a prodrug that the liver enzyme CYP2D6 partly turns into hydromorphone, the form that adds extra pain relief. Lab studies hint that sage may slow down CYP2D6. If that happened in people, it could mean a little less of hydrocodone's converted painkilling effect, though the drug still works through other paths.
Honestly, this is theoretical, seen only in test-tube studies and never reported in real patients. Cooking with sage almost certainly won't matter; concentrated extracts or the essential oil are the bigger question mark. The antihistamine (drowsiness) and decongestant pieces aren't really affected here. If you take sage supplements regularly, just give your pharmacist a heads-up.

In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the simple version. This combo product contains hydrocodone, which your body partly clears through a liver enzyme called CYP3A4. In lab tests (test tube studies only), sage seemed to slow that enzyme down a bit. In theory, that could let a little more hydrocodone build up than usual, which might mean stronger sedation, more drowsiness, dizziness, constipation, or slowed breathing.
The honest truth is this has never actually been reported in real people, and cooking with sage is very unlikely to cause any trouble. Concentrated sage capsules or oils are where I'd be more cautious. If you take sage supplements regularly while on this medicine, just mention it to your pharmacist or doctor so they can keep an eye on things.

ANTICHOLINERGIC DRUGS

In vitro evidence suggests that common sage (Salvia officinalis) and Spanish sage (Salvia lavandulaefolia) can inhibit acetylcholinesterase and might increase acetylcholine levels.

Here's the idea with Tussend (which combines chlorpheniramine, hydrocodone, and pseudoephedrine) and sage. One ingredient, chlorpheniramine, is an antihistamine that works partly by blocking a brain chemical called acetylcholine. Lab studies hint that sage may do the opposite, nudging acetylcholine levels up a bit. So in theory, concentrated sage could slightly blunt the antihistamine part, meaning a little less drying of a runny nose or watery eyes.
Honestly, this is very theoretical. It comes only from test-tube work, not from people, so I wouldn't lose sleep over it. A pinch of sage in your cooking is a non-issue. If you're taking strong sage extract capsules regularly, just mention it to your pharmacist so we can keep an eye on things.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
