Clozapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

Here's the gist with sage and clozapine. Your body uses a liver enzyme called CYP2D6 to help clear clozapine out. Lab studies (test tube only) hint that concentrated sage extracts might slow that enzyme down. If that happened in a real person, clozapine could build up a bit, which may mean more drowsiness, dizziness, drooling, a faster heartbeat, or low blood pressure.

The honest news: this is theoretical and has never actually been reported in people. Normal cooking amounts of sage are very unlikely to matter. Clozapine is a closely monitored medicine, though, so before using strong sage supplements or sage oil, check with your pharmacist or doctor first.

In vitro evidence suggests that common sage (Salvia officinalis) and Spanish sage (Salvia lavandulaefolia) can inhibit acetylcholinesterase and might increase acetylcholine levels.

Here's the idea behind this one. Clozapine has some anticholinergic activity, meaning part of how it works (and part of why it causes side effects like dry mouth, constipation, and grogginess) involves blocking a brain chemical called acetylcholine. In lab dishes, sage appears to do the opposite, nudging acetylcholine levels up a bit. So in theory, concentrated sage could work against clozapine's anticholinergic side and slightly blunt some of its effects.

I want to be honest with you, though: this is only test-tube evidence, not real patient data, so it's more of a maybe than a sure thing. A little sage as a cooking herb or in tea is very unlikely to matter. Strong sage extracts or sage oil are where I'd be more cautious. Clozapine is a closely monitored medicine, so please loop in your pharmacist or doctor before adding any sage supplement.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
