Codeine Phosphate, Pseudoephedrine, Triprolidine falls under CNS DEPRESSANTS — the group Sage interacts with.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Let's talk about CoActifed, which combines codeine, pseudoephedrine, and an antihistamine called triprolidine. The codeine and triprolidine parts can both make you drowsy. Some lab studies suggest a few compounds in sage have mild calming, sedative-like activity. So in theory, pairing concentrated sage with this medicine could add to the sleepiness, leaving you feeling groggier, foggy, or slower to react than usual. The pseudoephedrine is a stimulant, so it may offset some of that, but it's hard to predict.
Honestly, this is a theoretical concern based only on lab work, not on real patients. A little sage in your cooking is very unlikely to matter. Strong sage extracts or essential oil are where I'd be more cautious. If you take CoActifed regularly, just check with your pharmacist before adding sage supplements.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 2Codeine Phosphate, Pseudoephedrine, Triprolidine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

Here's the wrinkle with this combo: the codeine part of CoActifed is a prodrug. Your body has to convert it into morphine using an enzyme called CYP2D6 before it can ease your cough or pain. Lab tests suggest sage may slow that same enzyme down. If it did that in real life, less codeine would turn into morphine, so you might get weaker cough or pain relief rather than more side effects.
Honestly though, this is theoretical. It comes from test-tube studies only and hasn't been seen in actual people, and a sprinkle of sage in cooking is very unlikely to matter. Concentrated sage extracts or the essential oil are the bigger question marks. If you take those regularly, mention it to your pharmacist or doctor.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
