Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Sage interacts with.
Animal research suggests that common sage (Salvia officinalis) can cause prolonged blood pressure reduction. However, clinical research suggests that Spanish sage (Salvia lavandulaefolia) can increase blood pressure in some people with hypertension. Until more is known, use with caution.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi combines two medicines, macitentan and tadalafil, that both work to relax blood vessels and lower pressure in the lungs and body. That means your blood pressure is already being nudged downward on purpose. Sage is a wildcard here. Lab and animal studies hint it might lower blood pressure further, while other research on Spanish sage suggests it could actually raise it in some people. In short, we honestly don't know which way it will push.
This is theoretical and based on weak (lab/animal) evidence, so please don't panic. Sprinkling sage on your dinner is very unlikely to matter. Concentrated sage extracts or capsules are the real question. If your blood pressure runs low, dizziness, lightheadedness, or headache could get worse. Chat with your pharmacist or doctor before adding a sage supplement.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2C9. So far, this interaction has not been reported in humans.

Opsynvi is a combo pill of macitentan and tadalafil, both used to treat pulmonary arterial hypertension (high blood pressure in the lungs). The idea here is that sage, in lab dishes, can slow down a liver enzyme called CYP2C9 that helps clear these medicines. If that same effect happened in people, the drugs could build up a bit and their blood-pressure-lowering action could get stronger.
Practically, that might mean more dizziness, headache, flushing, low blood pressure, or swelling. But I want to be honest with you: this is only a theoretical concern from test-tube studies, and it has never actually been reported in humans. A little sage in your cooking is very unlikely to matter. Concentrated sage extracts or the essential oil are where I'd be more cautious, so please run those by your pharmacist or doctor first.

Opsynvi combines two medicines, macitentan and tadalafil, and both are handled in part by a liver enzyme called CYP3A4. In lab (test tube) studies, sage extracts appear to slow that enzyme down a bit. If that happened in the body, these drugs could clear a little more slowly and build up, which might nudge up their usual effects like low blood pressure, dizziness, headache, flushing, a stuffy nose, or leg swelling.
Here's the honest part: this is only a theoretical concern from laboratory work, and it hasn't actually been reported in people. Everyday cooking amounts of sage are very unlikely to matter. Concentrated sage extracts or the essential oil are a different story. Before adding a sage supplement, run it by your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven.
