Mestranol, Norethynodrel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

This older combination pill blends an estrogen (mestranol, which your body converts to ethinyl estradiol) with a progestin (norethynodrel). Both hormones are cleared in part by a liver enzyme called CYP3A4. Lab studies hint that water-based sage extracts may slow that enzyme down a bit, which in theory could let these hormones build up slightly higher than usual.

If that happened, you might notice more typical estrogen-type side effects like nausea, breast tenderness, bloating, or headaches. But honestly, this has only been seen in test-tube studies, never in real people, so I wouldn't lose sleep over it. A pinch of sage in your cooking is no concern. If you take concentrated sage capsules or oil daily, just mention it to your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory.
