Slippery Elm and Tadalafil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ORAL DRUGS

Why this applies to Tadalafil

Tadalafil falls under ORAL DRUGS — the group Slippery Elm interacts with.

Theoretically, slippery elm may slow the absorption and reduce serum levels of oral drugs.
Slippery elm inner bark contains mucilage, which may interfere with the absorption of orally administered drugs.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea with slippery elm and your tadalafil (Cialis): slippery elm forms a thick, gel-like coating in your stomach and gut. In theory, that slick layer could get between your tadalafil tablet and the gut wall, so a little less of the drug gets absorbed into your bloodstream. If your levels run lower, the medication might not work quite as well, meaning a weaker response for ED or less relief from BPH urinary symptoms.

I want to be honest about how soft this concern is. It's theoretical, based on how the fiber behaves, not on real cases in people taking tadalafil. A simple, practical fix is spacing them apart, taking slippery elm a couple of hours away from your tadalafil. Run that timing by your pharmacist or doctor first.

This page covers Slippery Elm only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Tadalafil page lists 144 supplements it has a documented interaction with, worst first, each one explained.

The most serious there right now: Activated Charcoal (major), American Mistletoe (major), Grapefruit (major), Hawthorn (major) and St. John's Wort (major).

By severity: 5 major · 98 moderate · 41 minor.

From our pharmacist’s overview on the Tadalafil page

“Grapefruit juice is the first thing to know about if you take tadalafil (Cialis, Tadliq). It blocks CYP3A4, the enzyme that clears the drug, so blood levels climb and side effects like a steep drop in blood pressure become more likely.”

Slippery Elm

  • Heartburn and acid reflux
  • Digestive upset and IBS
  • Skin irritation (topical)
  • Constipation or diarrhea

Slippery elm is a traditional herbal remedy made from the inner bark of a North American elm tree, used mainly to soothe sore throats and irritated digestive tracts. Its mucilage can coat and calm mucous membranes, but solid human evidence for most uses is limited. It is generally considered low-risk for short-term use, but you should still check with your pharmacist or doctor first.
