Cases of hepatotoxicity and hepatic failure have been reported in patients who ingested a large quantity (up to 100 grams) of Syrian rue seeds.

Here's the concern with pairing Syrian Rue and baricitinib (Olumiant). This isn't about Syrian Rue changing how your body activates or clears the drug. Instead, the worry is that both can be tough on the liver. Baricitinib can occasionally raise liver enzymes on its own, and large amounts of Syrian Rue seeds have been linked to liver injury and even liver failure. Stacking two liver stressors could, in theory, add up.

I want to be honest that this is a theoretical caution based on how these substances behave, not from studies of people taking them together. The reported liver problems came from swallowing very large quantities of seeds (up to 100 grams). Still, given the real toxicity risk, please check with your pharmacist or doctor before combining them.

What is Syrian Rue?

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. Talk to a pharmacist or doctor before considering it.

About Baricitinib

What Is Baricitinib Used For?

Brand name: Olumiant

Olumiant (baricitinib) tablets are approved for three conditions in adults. It treats moderately to severely active rheumatoid arthritis in people who haven't responded well enough to one or more TNF blocker medications. It also treats severe alopecia areata (significant hair loss from an autoimmune condition).

Olumiant is additionally approved to treat COVID-19 in hospitalized adults who need supplemental oxygen, mechanical ventilation, or ECMO (a machine that does the work of the heart and lungs). Each use has specific limitations — for example, Olumiant should not be combined with other JAK inhibitors or certain strong immunosuppressants for rheumatoid arthritis or alopecia areata.
