Hydrocodone, Ibuprofen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP2D6.
Syrian rue contains beta-carboline alkaloids, including harmine, harmaline, harmalol, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids can inhibit CYP2D6 enzymes.

Vicoprofen pairs hydrocodone, an opioid, with ibuprofen. The piece that interacts here is the hydrocodone half. Your body uses an enzyme called CYP2D6 to turn hydrocodone into hydromorphone, which carries a good share of its pain-relieving punch. Lab studies suggest the harmala alkaloids in Syrian Rue can block CYP2D6.

Blocking that enzyme could slow this conversion, so in theory you might get a bit less pain relief than expected. Just as important, Syrian Rue acts like an MAO inhibitor, and mixing MAOIs with opioids can cause dangerous reactions, so this combo deserves real caution. Keep in mind this CYP2D6 effect is theoretical and not proven in people. Please run it by your pharmacist or doctor before combining them.

Vicoprofen combines hydrocodone (an opioid pain reliever) with ibuprofen. The hydrocodone part is partly handled by a liver enzyme called CYP3A4. Lab studies suggest the harmala alkaloids in Syrian Rue can slow that enzyme down. In theory, that could let hydrocodone build up a bit more than usual, leading to stronger effects like extra drowsiness, dizziness, constipation, slowed breathing, or feeling foggy.

Keep in mind this is only a theoretical concern based on test-tube data, not real patient reports, so it may not amount to much. Still, Syrian Rue also acts as an MAO inhibitor and carries its own toxicity risks, which makes mixing it with an opioid a worthwhile thing to check first. Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment.
