Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Here's the basic idea with Adempas (riociguat). Your liver uses an enzyme called CYP3A4 to help clear this medication out of your body. Lab studies suggest that compounds in Syrian Rue may block that same enzyme. If that happens, riociguat could clear more slowly and build up to higher levels than expected.

Riociguat already lowers blood pressure, so more of it in your system could mean too much of that effect. Watch for things like dizziness, lightheadedness, feeling faint when you stand up, headache, or a fast heartbeat. To be fair, this concern comes from test-tube research only, not real patients, so it's theoretical. Still, since blood pressure drops can be serious, I'd run it by your pharmacist or doctor before combining them.

What is Syrian Rue?

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
