Roflumilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Here's the gist for roflumilast (Daliresp, Zoryve): your liver uses an enzyme called CYP3A4 to help clear it. Lab studies suggest the harmala alkaloids in Syrian Rue can slow that enzyme down. If that happens, roflumilast could build up higher than usual, which tends to amplify its well-known side effects, things like nausea, diarrhea, weight loss, headache, trouble sleeping, and sometimes mood changes.

I'll be straight with you though: this is a theoretical concern based on test-tube data, not real-world cases, so I don't want to alarm you. Still, Syrian Rue carries its own toxicity risks and isn't recommended for self-treatment. Please check with your pharmacist or doctor before combining the two.

What is Syrian Rue?

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. Talk to a pharmacist or doctor before considering it.

About Roflumilast

What Is Roflumilast Used For?

Brand names: Daliresp, Zoryve

Daliresp and roflumilast tablets are used to reduce the risk of COPD (chronic obstructive pulmonary disease) flare-ups in adults with severe COPD linked to chronic bronchitis who have had flare-ups in the past. They are not rescue inhalers and will not relieve a sudden breathing attack.

Zoryve cream is used topically to treat plaque psoriasis (adults and children 2+) and mild to moderate atopic dermatitis (eczema) in adults and children as young as 2. Zoryve topical foam is used to treat seborrheic dermatitis (adults and children 9+) and plaque psoriasis of the scalp and body (adults and children 12+).
