Roflumilast (Daliresp for severe COPD, Zoryve as a cream for skin conditions) is cleared almost entirely by one liver enzyme, CYP3A4, which is why 121 of the 141 supplements on this page can push its level up or down, and why real caution is warranted here. Start with the three rated major: grapefruit blocks that enzyme and can raise roflumilast into side-effect territory, while St. John's wort revs the enzyme up and can quietly drain the drug's benefit.

The third major, activated charcoal, works differently, binding the drug in the gut so less ever reaches the bloodstream. A smaller group of 17 entries, fibers like glucomannan among them, mainly blunts absorption of oral doses, and spacing them well apart usually settles it. Nearly all of the 96 moderate and 42 minor entries are fainter versions of the same enzyme story, but grapefruit and St. John's wort are the two I would give up outright.
