Roflumilast falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the quick version: your body clears roflumilast partly through a liver enzyme called CYP3A4. In some lab studies, bacopa appears to gently slow that same enzyme down. If that happened in real life, roflumilast could build up a bit and lead to more of its usual side effects, things like nausea, diarrhea, headache, trouble sleeping, reduced appetite, or some weight loss.

The honest part: this concern comes only from test-tube studies, and even those disagree with each other. So this is more of a theoretical caution than a proven problem, and many people would notice nothing. Still, since roflumilast can be hard on the stomach already, it's worth mentioning bacopa to your pharmacist or doctor before combining them, and let them know if side effects pick up.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Roflumilast Used For?

Brand names: Daliresp, Zoryve

Daliresp and roflumilast tablets are used to reduce the risk of COPD (chronic obstructive pulmonary disease) flare-ups in adults with severe COPD linked to chronic bronchitis who have had flare-ups in the past. They are not rescue inhalers and will not relieve a sudden breathing attack.

Zoryve cream is used topically to treat plaque psoriasis (adults and children 2+) and mild to moderate atopic dermatitis (eczema) in adults and children as young as 2. Zoryve topical foam is used to treat seborrheic dermatitis (adults and children 9+) and plaque psoriasis of the scalp and body (adults and children 12+).
