Why this applies to Thioridazine

Thioridazine falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C19 substrates.
In vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C19 enzymes. It is not known whether this is clinically significant.

Here's the gist with thioridazine (Mellaril) and bacopa. Your body relies in part on a liver enzyme called CYP2C19 to clear thioridazine out. In test tube studies, bacopa seems to slow that enzyme down a bit. If that holds true in real people, thioridazine could build up higher than expected, which may bring on more of its known side effects like drowsiness, dizziness, dry mouth, low blood pressure, or heart rhythm changes (this drug already carries a warning about a rhythm problem called QT prolongation).

I'll be honest though: this is only laboratory evidence, so we don't truly know if it matters in everyday use. Because thioridazine is a touchy medication, please run this by your pharmacist or doctor before adding bacopa rather than changing anything yourself.

Theoretically, concurrent use might decrease the effectiveness of both agents.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.

Here's the idea behind pairing bacopa with thioridazine (Mellaril). Thioridazine is an older antipsychotic that carries anticholinergic activity, meaning it dampens a brain chemical called acetylcholine. Bacopa seems to do the opposite in lab studies, gently nudging acetylcholine levels up. So in theory, the two could work against each other and slightly blunt one another's effects.

I'll be honest with you: this concern is theoretical, drawn from how each works rather than real patient reports. Any change would likely be small. Still, thioridazine is a serious medication you've earned a stable dose of, so I wouldn't add bacopa without first checking with your psychiatrist or pharmacist. Please don't change either one on your own.

Bacopa is an Ayurvedic herb most often used for memory and thinking. What Is Thioridazine Used For?

Brand name: Mellaril

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.

Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
