Acetaminophen, Caffeine, Dihydrocodeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill contains caffeine, which your body clears mainly through a liver enzyme called CYP1A2. In test-tube studies, bacopa might slow that enzyme down a little. If it does, caffeine could hang around longer and build up, which may leave you feeling jittery, restless, anxious, or having trouble sleeping, plus maybe a faster heartbeat.
The good news: this is only a theoretical concern from lab work, and the research actually conflicts, so a real-world effect is far from certain. The acetaminophen and dihydrocodeine parts aren't really driven by this pathway. If you take bacopa regularly and notice extra caffeine-like jitters, mention it to your pharmacist or doctor before changing anything.

Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo pill includes dihydrocodeine, an opioid that the liver partly handles through an enzyme called CYP3A4. In lab dishes, bacopa has shown a mild ability to slow that same enzyme, though the studies disagree and some suggest the effect is too small to matter in real life. The concern here is only theoretical.
If bacopa did slow that enzyme even a little, dihydrocodeine could build up slightly and lead to more of its usual effects, like drowsiness, dizziness, constipation, or slowed breathing. The caffeine and acetaminophen parts aren't the main worry. Since this is based only on test-tube data, I wouldn't panic, but it's worth a quick word with your pharmacist or doctor before starting bacopa, especially if you feel extra sleepy.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.
