These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Bacopa interacts with whole categories of medication. Belladonna, Ergotamine, Phenobarbital falls into 2 of those categories, so it can interact with Bacopa in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Belladonna, Ergotamine, Phenobarbital
Belladonna, Ergotamine, Phenobarbital falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bacopa interacts with.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Bellergal-S, which combines belladonna, ergotamine, and phenobarbital. Lab studies hint that bacopa might mildly slow down a liver enzyme called CYP3A4, which helps clear the ergotamine part of this medicine. If that clearing slows, ergotamine could build up a little, and that's the piece I'd watch, since too much ergotamine can cause nausea, tingling or coldness in the hands and feet, leg cramps, or headache.
That said, this is only a theoretical concern. The lab evidence is mixed, and some of it suggests no real-world effect at all, so there's no need to panic. Still, before pairing them regularly, it's worth a quick chat with your pharmacist or doctor to be safe.

ANTICHOLINERGIC DRUGS
2 of 2Belladonna, Ergotamine, Phenobarbital also falls under ANTICHOLINERGIC DRUGS.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of Bellergal-S that matters here is the belladonna, which is an anticholinergic. It works by blocking acetylcholine, the very chemical messenger that bacopa may nudge upward. On paper, that means the two could partly cancel each other out. In simple terms, bacopa might slightly weaken belladonna's calming, drying effects, and belladonna might blunt some of what bacopa is trying to do for memory.
I want to be honest though: this is a theoretical concern based on how these things behave in the lab, not something we see causing real problems in people. It's worth a quick mention to your pharmacist or doctor, especially since this combo also contains ergotamine and phenobarbital, but there's no need to panic over it.

From our pharmacist’s overview on the Belladonna, Ergotamine, Phenobarbital page
“Bellergal-S packs three drugs into one tablet, and its supplement list earns real respect before you mix in anything else. Phenobarbital is a sedating barbiturate, belladonna adds drying anticholinergic effects, and ergotamine constricts blood vessels, so this combination collects interactions from several directions at once.”
