Tofacitinib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C19 substrates.
In vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C19 enzymes. It is not known whether this is clinically significant.

Here's the idea behind this pairing. In lab dishes, bacopa appears to mildly slow down a liver enzyme called CYP2C19, which is one of the pathways your body uses to clear tofacitinib (Xeljanz). If that enzyme is held back, a little more of the drug could linger in your system, which could nudge up its effects and side effects, things like headaches, diarrhea, upper respiratory infections, or a higher chance of infection in general.

I want to be honest about how thin this evidence is. It comes only from test-tube studies, not from people taking bacopa and tofacitinib together, so we truly don't know if it matters in real life. Food-style amounts are very unlikely to be an issue, but concentrated supplements are the bigger question. Please don't change anything on your own. Just run it by your pharmacist or doctor first.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the basic idea: your body uses a liver enzyme called CYP3A4 to help clear tofacitinib (Xeljanz) out of your system. In lab dish studies, bacopa has shown it might slow that enzyme down a bit. If that happened in real life, more tofacitinib could linger in your blood, which could nudge up its effects and side effects, things like headaches, higher cholesterol, or a greater chance of infections since this drug already dampens the immune system.

The honest truth is this is theoretical. The only evidence comes from test tube studies, and even those conflict, so we don't know if it matters in people. I wouldn't lose sleep over it, but since tofacitinib is a serious medication, please run any bacopa use past your pharmacist or doctor first.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Tofacitinib Used For?

Brand names: Xeljanz, Xeljanz XR

  • Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
