CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Tofacitinib
Tofacitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body partly clears tofacitinib (Xeljanz) using a liver enzyme called CYP3A4. Tofacitinib is active as-is, so it is not a prodrug. Quercetin in high-dose supplement form may slow that enzyme down in some people, which could let a bit more tofacitinib build up. In theory that might mean stronger effects and a higher chance of side effects like headaches, diarrhea, upper-respiratory infections, or other signs your immune system is being dialed back too far.
I want to be honest, though: this is mostly based on lab and animal work, and the human data is mixed (quercetin has even sped up CYP3A4 in one study). Food amounts of quercetin are unlikely to matter. Before starting a concentrated supplement, just run it by your pharmacist or doctor.

What Is Tofacitinib Used For?

Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
