Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP2C9 substrates.
Research on the effect of bacopa extracts on CYP2C9 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and non-competitively inhibit CYP2C9, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist: your body relies on a liver enzyme called CYP2C9 to help break down bosentan (Tracleer). In lab dish studies, bacopa has shown a possible ability to slow that enzyme down a bit. If that happened in a real person, bosentan could clear more slowly and build up, which might mean more of its usual side effects like headache, flushing, swelling in the legs, or changes in liver-related blood tests.

The honest part: this is only theoretical and based on test-tube research, and even that evidence is mixed. We don't have human studies showing a real problem. Still, since bosentan needs careful liver monitoring, it's worth a quick chat with your pharmacist or doctor before adding bacopa rather than starting it on your own.

Here's the gist for your Tracleer (bosentan): your liver uses an enzyme called CYP3A4 to help clear this drug from your body. In some lab dish studies, bacopa appeared to mildly slow that same enzyme down. If that happened in real life, bosentan levels could creep up a bit, which might mean more of its usual side effects, things like headache, flushing, swelling in the legs, or feeling tired.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan treats pulmonary arterial hypertension (PAH), which is high blood pressure in the arteries of the lungs (WHO Group 1). In adults, it helps improve exercise ability and slows clinical worsening.

In children 3 years and older with idiopathic or congenital PAH, it is used to improve pulmonary vascular resistance (the resistance blood meets in the lung vessels). This is expected to improve exercise ability. These uses appear on the Tracleer label and on the bosentan tablet and tablet for oral suspension labels.
