Here's the thing to know about pairing St. John's wort with Bosentan (Tracleer). Your liver uses an enzyme called CYP3A4 to clear bosentan from your body. St. John's wort revs up that enzyme, so it can push bosentan out faster and leave less of it working in your system.

For a medicine treating pulmonary arterial hypertension, that's a real concern. Lower bosentan levels may mean it doesn't control the high pressure in your lung vessels as well, so you could notice your breathing or exercise tolerance slipping. Bosentan isn't a prodrug, so this herb tends to weaken its effect, not boost it. Please don't change anything on your own. Talk with your pharmacist or doctor before combining these two.

Bosentan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

St. John's wort may increase the metabolism and reduce the levels of CYP2C9 substrates.
There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Here's the gist for bosentan (Tracleer). Your body partly clears this medicine through liver enzymes, and St. John's wort tends to rev those enzymes up. When the enzymes work faster, more bosentan gets broken down, so the amount in your blood can drop. That could mean it works less well at keeping your lung blood vessels open, which is the whole point of taking it for pulmonary arterial hypertension.

One twist worth knowing: bosentan itself also nudges these same enzymes, so the two together get complicated. The research here is mixed and not super strong, so I wouldn't panic. But because keeping PAH controlled matters a lot, please don't combine these without checking first. Talk with your pharmacist or doctor before adding St. John's wort.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
