Bosentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the short version on mixing higenamine with bosentan (Tracleer). Your body clears bosentan mainly through a liver enzyme called CYP3A4. In a lab dish, higenamine slowed that enzyme down a bit (about 21%). If that happened in people, bosentan could build up a little, which might mean more of its usual side effects like headache, flushing, swelling in the legs, or feeling tired or lightheaded.

The big caveat: this is only test-tube data and has never been shown in actual humans, so it's theoretical. Still, I'd be cautious. Higenamine itself can raise heart rate and blood pressure, which isn't ideal when you're already managing pulmonary hypertension. Please loop in your pharmacist or doctor before combining them.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
