Why this applies to Bosentan

Bosentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Tracleer (bosentan). Your body relies on a liver enzyme called CYP3A4 to clear bosentan out of your system. Lab studies hint that lentinan may speed that enzyme up. If that happened in real life, it could push bosentan out faster, leaving less of it working in your blood vessels. The worry would be that your PAH isn't controlled as well, so you might notice more shortness of breath or trouble keeping up with activity.

That said, I want to keep this in perspective. This signal comes only from test-tube and animal research, and it has never actually been seen in people. So it's something to be aware of, not to panic over. Before adding lentinan, just run it by your pharmacist or doctor so they can keep an eye on how you're feeling.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
