Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the gist for Bosentan (Tracleer): your liver uses an enzyme called CYP2C9 to help clear it from your body. Ipriflavone may slow that enzyme down a bit. In theory, that could let bosentan build up to higher levels than usual, which might mean more of its typical side effects, things like headache, flushing, swelling in the legs or ankles, low blood pressure, or signs of liver irritation.

The good news is this is theoretical, coming from lab studies rather than real-world patient reports, so it's not a sure thing. Bosentan already needs regular liver blood tests, so I wouldn't add ipriflavone on your own. Just check with your pharmacist or doctor first so they can decide if it's a good fit for you.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
