Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Jambolan interacts with.

There is some in vitro evidence that jambolan can inhibit CYP2C9. Theoretically, concomitant use of jambolan with CYP2C9 substrates may increase levels of drugs metabolized by CYP2C9.

Here's the gist with bosentan (Tracleer) and jambolan. Your body relies on a liver enzyme called CYP2C9 to help clear bosentan out of your system. Lab studies hint that jambolan might slow that same enzyme down. If that happens in real life, bosentan could build up a bit more than usual, which may mean more side effects like headache, flushing, swelling in the legs, or feeling unusually tired.

I want to be honest about the strength of this, though. So far it's only test-tube evidence, not studies in actual patients, so this is more of a theoretical caution than a proven problem. Eating the fruit as food is unlikely to matter much. Concentrated supplements are the bigger question mark. Since bosentan needs regular liver monitoring anyway, check with your pharmacist or doctor before adding a jambolan supplement.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
