Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist for Orgovyx (relugolix). Your body partly clears this medication using a liver enzyme called CYP3A4. In lab dish studies, bacopa has shown it might slow that enzyme down a little. If that happened in real life, relugolix could build up slightly, which in theory could mean more of its usual effects, like fatigue, hot flashes, or joint aches.

I want to be honest about the strength of this: the evidence is weak and conflicting, coming only from test-tube studies, and some of it suggests no meaningful effect at all. So this isn't a reason to panic. Still, since relugolix treats prostate cancer, it's worth a quick mention to your oncologist or pharmacist before adding a bacopa supplement, just so everyone's on the same page.
