Relugolix falls under QT INTERVAL-PROLONGING DRUGS — the group Arsenic interacts with.

Arsenic trioxide can prolong the QT interval.
Up to 40% of patients treated with prescription arsenic trioxide have a prolonged QT interval on their electrocardiogram (ECG). Theoretically, non-prescription arsenic could have an additive effect when combined with drugs that prolong the QT interval.

Here's the concern with this pairing. Relugolix (Orgovyx) can stretch out a part of your heart's electrical cycle called the QT interval. Arsenic, in its prescription form (arsenic trioxide), is well known to do the same thing, in fact up to 40% of people on it show a longer QT on their ECG. Putting two QT-stretching things together can add up, and a very long QT can lead to a dangerous, irregular heartbeat.

The good news is Relugolix is not a prodrug, so this isn't about how it's broken down, it's just an overlapping heart effect. Also, the worry about non-prescription arsenic is mostly theoretical. Still, there's no reason to take arsenic on purpose. Please loop in your doctor or pharmacist with any concerns.
