Propranolol Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist with bacopa and propranolol. Propranolol is partly cleared by a liver enzyme called CYP1A2, and bacopa might slow that enzyme down a little. In theory, that could let more propranolol build up. Since this oral solution is usually given to babies for hemangiomas, the things to watch for would be a slower heart rate, lower blood pressure, sluggishness or unusual sleepiness, poor feeding, or low blood sugar.

That said, I'd keep this in perspective. The evidence here is weak (only lab and animal studies), and even those results disagree about whether bacopa affects this enzyme in any real way. Before giving an infant bacopa alongside propranolol, please run it by your pediatrician or pharmacist first.

Here's the gist: bacopa, in test-tube studies only, seems to mildly slow down a liver enzyme called CYP2C19, which is one of the pathways that helps clear propranolol from the body. In theory, that could let a bit more propranolol build up. For your little one, more drug effect could mean things like a slower heart rate, low blood pressure, or low blood sugar showing up as sleepiness or poor feeding.

I want to be honest though, this is only a laboratory finding, and we don't know if it actually matters in real life. It's also unusual to give an infant on Hemangeol a bacopa supplement at all. Please don't change anything on your own. Just run it by your child's doctor or pharmacist first.

Bacopa is an Ayurvedic herb most often used for memory and thinking. What Is Propranolol (Infantile Hemangioma) Used For?

Brand name: Hemangeol

This intravenous (IV) form of propranolol is used in hospital settings to treat serious, life-threatening heart rhythm problems. It works on several types of abnormal rhythms, including those that happen during anesthesia or are caused by drug toxicity.

Specific uses include fast heart rhythms coming from the upper chambers of the heart (supraventricular tachycardia), dangerous rhythms caused by too much digitalis in the body, and certain fast rhythms triggered by excess stress hormones (catecholamines) during surgery.
