Here's the gist for Fycompa (perampanel). Your liver uses an enzyme called CYP1A2 to help clear this seizure medicine. In lab studies, bacopa may slow that enzyme down a bit. If that happened in real life, perampanel could build up slightly, and you might notice more of its usual side effects, like dizziness, sleepiness, trouble with balance, or feeling irritable or moody.

I want to be honest about the strength of this, though. It's theoretical, based only on test-tube and animal data, and the studies actually disagree with each other. So this is more of a heads-up than a red alert. Since perampanel is a seizure medicine where steady levels matter, just loop in your pharmacist or doctor before adding bacopa, and mention any new drowsiness if you do.

Here's the basic idea with bacopa and Fycompa (perampanel). Lab studies hint that bacopa can mildly slow down a liver enzyme called CYP2C19, which is one of the pathways your body uses to clear perampanel. If that enzyme is working slower, perampanel could build up a bit more than expected. Since perampanel isn't a prodrug (it's already active on its own), more in your system generally means stronger effects and a higher chance of side effects like dizziness, sleepiness, trouble with balance, irritability, or mood changes.

That said, this concern comes only from test-tube research, not real patients, so we honestly don't know if it matters in everyday life. I wouldn't panic, but do mention bacopa to your doctor or pharmacist before combining them, especially since perampanel already affects alertness.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
