Bacopa and Bendamustine Hydrochloride

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Bendamustine Hydrochloride

Bendamustine Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this pairing. Bendamustine (Bendeka) is partly broken down by a liver enzyme called CYP1A2. In lab dish studies, bacopa has shown a possible ability to slow that enzyme down. If that happened in your body, bendamustine could clear a little more slowly and build up, which in theory could mean stronger side effects like low blood counts, nausea, fatigue, fever, or a higher infection risk.

I want to be honest about the strength of this, though. This concern comes only from test-tube and animal research, and even those results conflict. It hasn't been shown to be a real problem in people. Still, since bendamustine is a serious chemotherapy drug, please run any supplement, including bacopa, by your oncology team before using it.

From our pharmacist’s overview on the Bendamustine Hydrochloride page

“Bendamustine (Bendeka), a chemotherapy used for certain leukemias and lymphomas, carries 86 supplement and herbal interactions on file, and none of them rates major, so this is a watch-and-adjust list rather than an alarm. Nearly all of the entries, about 80, involve the same liver enzyme, CYP1A2, which helps process bendamustine.”
