Bacopa and Acetaminophen, Phenylephrine, Chlorpheniramine

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Bacopa interacts with whole categories of medication. Acetaminophen, Phenylephrine, Chlorpheniramine falls into 3 of those categories, so it can interact with Bacopa in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Acetaminophen, Phenylephrine, Chlorpheniramine

Acetaminophen, Phenylephrine, Chlorpheniramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bacopa interacts with.

Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist with your cold combo (acetaminophen, phenylephrine, and chlorpheniramine). The concern is that bacopa might gently slow down a liver enzyme called CYP1A2, which can play a role in clearing certain medicines. If a drug clears more slowly, it can hang around a bit longer. In theory, that could nudge up effects like the drowsiness and dry mouth from chlorpheniramine, or the jittery, racy feeling from phenylephrine.

That said, this is very shaky evidence, based only on lab studies, and even those results disagree with each other. None of these three ingredients depend heavily on CYP1A2, so real-world trouble is unlikely. Still, if you take bacopa regularly, mention it to your pharmacist so they can double-check for you.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 3

Acetaminophen, Phenylephrine, Chlorpheniramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, bacopa might increase the levels and adverse effects of CYP3A4 substrates.
Research on the effects of bacopa extracts on CYP3A4 enzymes is conflicting. Some in vitro evidence suggests that bacopa extract can moderately and competitively inhibit CYP3A4, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist for your Super Cold Tabs combo. In a test tube, bacopa might mildly slow down a liver enzyme called CYP3A4, which helps break down certain medicines. If that happened in your body, parts of this cold tablet could hang around a little longer and feel stronger. That could mean more drowsiness or dry mouth from the chlorpheniramine, or a bit more of the jittery, blood-pressure-raising effect from the phenylephrine.

That said, the evidence here is weak and only comes from lab studies, and other research suggests it probably doesn't matter much in real life. None of these ingredients is a prodrug, so we're talking about a possible small bump in effect, not treatment failure. Take an occasional cold tablet as directed and keep an eye on how you feel. If you use bacopa daily, just mention it to your pharmacist or doctor.

ANTICHOLINERGIC DRUGS

3 of 3

Acetaminophen, Phenylephrine, Chlorpheniramine also falls under ANTICHOLINERGIC DRUGS.

Theoretically, concurrent use might decrease the effectiveness of both agents.
Bacopa seems to inhibit acetylcholinesterase and might increase acetylcholine levels, which could counteract the effects of anticholinergic drugs. Similarly, anticholinergic drugs might counteract the cholinergic effects of bacopa.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

The piece of your cold tablet that matters here is the chlorpheniramine, an antihistamine that works partly by blocking acetylcholine, one of the body's chemical messengers. Bacopa seems to push acetylcholine levels up, so in theory the two could work against each other. Practically, that might mean the chlorpheniramine doesn't calm a runny nose or sneezing quite as well, and bacopa's memory effects could also be blunted.

I want to be honest that this is a theoretical concern based on how these things work in the lab, not something well documented in real patients. For a short-term cold product taken a few days, it's unlikely to cause trouble. The acetaminophen and phenylephrine aren't involved. If you take bacopa regularly, just mention it to your pharmacist or doctor.

About Bacopa

What is Bacopa?

Bacopa
  • Memory and cognitive support
  • Anxiety and stress
  • ADHD symptoms
  • Age-related memory decline
  • General brain health

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.
